SFA SCHOOL REGISTRATION
Registration for students new to St. Francis of Assisi School
may be done by contacting the Business Office of the Primary Building at 112 Saxer
Avenue (610-543-0546). YOU MUST BE REGISTERED IN SFA PARISH BEFORE YOU CAN
REGISTER IN SCHOOL.
Registration fee is $90.00 /child - payable at registration.
This fee is non-refundable.
YOU MUST BRING
(1) BIRTH CERTIFICATE, (2) BAPTISMAL CERTIFICATE
(if child was not baptized at SFA) AND ALL INFORMATION
REQUIRED BY THE SCHOOL NURSE
Remember that this registration is only for students new
to St. Francis. Parents of SFA Kindergarten children need not
register their children for first grade at this time.
St.
Francis of Assisi School is fully accredited by the Middle
States Association of Colleges and Schools and provides an
outstanding academic education, rooted in Gospel values, where
every child is valued as an integral part of the whole school
community.

Please be advised that NO
registration will be accepted without proof of proper
immunization. Please check with your child's doctor to see if
your child has had the required immunizations. They are: 4
doses of DPT, 3 doses of Polio, 3 doses of Hepatitis B, 2 MMR,
Varicella Vaccine (Chickenpox) or proof they had the disease.
Proof of immunization must be in the form of a printed document
from the doctor, complete with the doctor's signature or stamp.
NO OTHER FORM WILL BE ACCEPTED. |
